Director of Care & Disease Management, Health Plan of San Joaquin, Stockton, CA
About the position:
Reporting to the Associate Medical Director, the Director will play a key role in further developing the care management and disease management functions and strengthening coordination between Utilization Management and Quality Improvement. In addition, the successful candidate will continue to help prepare the plan for its expected growth, which includes implementing a new care management software system, promoting EMR use and Medical Home development. The Director will develop a cohesive, fully engaged team that has a firm understanding of their role in helping the organization achieve its mission and strategic goals.
